The Center House Transition Program is intended for students who have a diagnosed disability with mild to moderate delays in cognitive abilities and adaptive skills. The students are at least 18 years of age and have completed four years of high school, including all graduation requirements.  Students are referred by the district high school programs in their junior or senior year of high school.  Students who attend may choose to stay until the day before their twenty-second birthdays.
